Client Care Coordinator--SUD Treatment

Description:
The Client Care Coordinator is responsible for the coordination of admissions and case management serviceswithin a behavioral health facility.
Under the supervision of the Program Manager, the Client Care Coordinatorcompletes assessments, and provides case management according to the needs of the client.
The Client CareCoordinator acts as a liaison between the multidisciplinary team and other departments, families and other outsideagencies to advocate for the client and remove barriers to care.
The Client Care Coordinator arranges dischargeand after care services with guidance from the clinical team.
The Client Care Coordinator builds relationships withcommunity partners to provide clients with referral and linkage to community resources.
Coordinates admissions with the Intake Department and facility2.
Completes admission paperwork3.
Completes behavioral assessments such as the Admission Assessment, Case Management Assessment, andother screens and assessments, within their scope, as needed4.
Provides orientation services to acclimate client to milieu5.
Provides case management services for client, which includes coordination of benefits with insurancecompanies, and may include providing assistance with filing for disability, food stamps, FMLA and otherservices, and/or communicating with attorneys, probation and parole officers, employers and other entities6.
Coordinates discharge and after care services with guidance from the clinical team7.
Assists with AMA/ACA (Against Medical Advice/Against Clinical Advice) blocking and client management8.
Liaises with the on-site multi-disciplinary team, as well as the Utilization Review Team, BusinessDevelopment and other departments9.
Leads groups in support of programming needs10.
Participates in case staffing and reports concerns11.
Attends daily staff meetings and weekly department meetings as pertinent to job function and responsibilities.

Assist staff in coordinating all admissions and dischargesPerformance Objectives1.

Works with administration to enhance the treatment environment4.

Possesses treatment knowledge related to Substance Use Disorders and Mental Health conditions.

Requirements:
Education:
High School Diploma or bachelor's degree
Experience:
1 year in Substance Abuse fieldLicensure/Certification:
Certification in addictions preferredKnowledge and Training Required:
State Specific Guidelines and Joint Commission Standards preferredAge Specific of Individuals Served:
Adults, 18 and older.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
